Thank you! No, I didn't use a template. I did follow a tutorial by June at Cen's Loft blog for making the wavy shape, and then I clipped the checked paper to it. Fun!

Ugh! Dunderhead alert! I credited the wrong tutorial in my comment above. :-( The correct tutorial information is located at Gotta Pixel under the "Ask Lisa" category ("How do I make wavy paper?"). I'm so sorry, I had a brain fart!
